Visit the HealthMark Group portal
Go to requestmanager.healthmark-group.com
Select "Submit Request"
Choose this option from the main navigation menu
Complete all required fields
Fill in your patient information and what records you need
Receive your notification
You’ll be notified by mail or email when records are ready

What’s Included?
Medical records and X-ray images can both be requested through the HealthMark Group portal.

Submit a signed authorization form directly to our team.
Authorization Form Required
Pain Center patients must submit a signed “Authorization for Release of OrthoConnecticut Medical Records” form along with their request.
